Traveling outside of the U.S?
The Customer Service Center is an authorized U.S. Government Passport Acceptance Facility and accepts the DS-11 application for the US Department of State. We offer products and services for both first-time and renewal passport applications.
Passport services are available to everyone – you do not need to be affiliated with UC Berkeley to take advantage of these services.

Hours of Operation
The Customer Service Center is currently open Monday – Friday, 9:30 am – 6:00 pm (PST.) and Saturday 9:30am – 4:00pm on an appointment-only basis; no walk-ins. We are unable to take Passport Photos at this time
Appointments are limited to 30 calendar days in advance. If fully booked, please continue checking as appointments will become available.
Administrative Holidays: Please note, passport services will be unavailable on all UC Berkeley administrative holidays. Our office will be closed March 28th-29th, March 31st and May 29th (2023)
Apply for your passport several months before your trip. First-time applicants, minors, and applicants who need a new passport but may not renew by mail may submit their passport application at the Customer Service Center. The U.S. State Department website explains what you'll need to bring with you.
Application forms (unsigned)
Valid Government-issued ID and photocopy of front and back.
Proof of U.S. Citizenship document and a photocopy.
DO NOT SIGN YOUR APPLICATION. A Customer Service Center employee must witness your signature during your appointment.
Have a passport photo taken.
Photocopy your proof of identity and U.S. Citizenship documents.
Calculate your fees (See Step 4).
Please note, in accordance with US Department of State regulations, the passport application fee must be paid by personal check, cashier’s check, or money order. Separate checks or money orders are required for each passport application. Checks must have the check maker’s name printed on them. The Department of State will not accept blank or starter checks.
By State Department mandate, each DS-11 processed is subject to a separate $35 fee payable to the Acceptance Facility - this can be paid with a credit or debit card.

Step 4: Pay Passport Acceptance & Processing Fees

DS-11 passport applications, the only form we accept have two types of basic fees, the application acceptance fee, and the application processing fee. Fees for expedited processing are extra.
DS-82 passport renewal applications to be mailed in by the applicant to the Department of State.
Application Processing Fees - Department of State
State Department payment is sent with your application package. State Department fees are paid separately from Customer Service Center fees.
State Department Payment methods: NOTE - You may not pay State Department fees with credit or debit cards. You may pay with:
Personal, certified, or cashier's check, payable to "U.S. Department of State."
Money order payable to "U.S. Department of State."

Pay acceptance fees in person at the Customer Service Center. You may pay by check, payable to "UC Regents", cash debit card, or credit card.

Passport Renewals

The Customer Service Center is able to process passport renewals for (1) minors or (2) those who are not minors and have lost their most recent passport via the DS-11. You must prepare the same application package as a first-time passport applicant.
We are processing passport sales and renewals on an appointment-only basis, no walk-ins. Please make an appointment before heading to the Customer Service Center.

You may renew your passport by mail if you still have your most recent passport. Certain conditions apply.
The passport must be:
In your current name (name changes are explained on Change a Passport)
Issued within the last 15 years
Issued when you were age 16 or older
Undamaged other than normal "wear and tear"
Submitted with your renewal application
If your passport does not meet all these requirements, do not mail your renewal application.
Passports for Minors Under 16
Passport applications for children under 16 have special requirements.
The applicant (minor) and both parents/guardians must be present to authorize the issuance of a child’s passport. Please visit the State Department website, item #7, if both parents/guardians cannot appear in person.
Children need evidence of citizenship.
Parents/guardians need proof of parental relationship.
Note: Children’s passports are only valid for 5 years.
